I grant permission to Envision Dance Company and its agents or employees, to use photographs of me, or the minor named below for use in studio publications including brochures, newspapers, official Envision Dance Company Social Media, and Envision Dance website.

I grant Envision Dance Company the unrestricted right to use and publish photograph or video footage taken of me or the minor listed below while participating in dance class, performing or in the company Envision Dance Company. I waive any rights to royalties or other compensation arising from the use the photograph or video footage of myself, or if signing for the minor listed below, I waive all rights of the minor listed below.

I agree on my own behalf or on the behalf of the minor listed below, to release and hold harmless Envision Dance Company and its agents or employees from any claims arising from the related use of the photographs. I acknowledge that by signing this form I give Envision Dance Company full copyright and authority to publish photography or video footage and I agree to the terms listed above.
